Your Personal Details:
During our initial meeting which will be in person, on a one-to-one basis, at your place of residence or a suitable venue of your choice, I will collect some personal information such as name and contact details, family circumstances, occupation, any relevant psychological diagnosis or medication, and GP.
How information is stored:
Very brief, anonymised, notes handwritten by me, will be kept in order to track the sessions. Your identifiable signed contract, personal and contact details, will be kept in a separate locked cabinet.
Electronic storage:
I keep minimal information on my work smartphone and computer, both of which are password protected. I will generally contact you either through my work smartphone by text or direct calling, or by email. I cannot guarantee the security of information communicated by email, or by mobile phone (voicemail, text messages or WhatsApp).
Details held and retention period:
Information will be kept for 7 years after the close of Coaching and/or Counselling. On the date of destruction your notes and any additional information will be securely destroyed.
How I may share your personal information:
In line with my professional guidelines, I have regular contact with a practitioner who is also bound by the same confidentiality regulations, as I am. During any such contact only my Client’s first name is ever used. Only myself, or if anything happens to me, my contact practitioner, will have access to Client details.
Our work will at all times be confidential, unless I observe something, or you tell me something, that I am legally obliged to report -such as the disclosure of details of drug trafficking, the abuse of children or vulnerable adults, money laundering or acts of terrorism.
If I am concerned that there is a significant risk of serious harm to yourself, or to someone else, I reserve the right to breach confidentiality in order to inform a relevant professional third party. In the first instance however, I would always seek to gain your consent before doing so.
Right to access:
You have a right to ask for a copy of your personal information, free of charge. You also have the right to ask me to amend or update any information about you. Should your notes be requested by a third party (i.e. solicitor), I will require a signed declaration from yourself. I will not hand over any notes without your permission, although I may be obliged to hand over notes if I am ordered to by a court. In this unlikely event, before any notes were sent, I would recommend to you that we meet up in person, or if this is not possible on-line, and explore the situation.
Appointments and Fees:
Appointments will finish at the scheduled time. The fee is payable on the say by cash, cheque, or bank transfer (if transfer details have been arranged prior to the day). Please note that I charge for cancellations or missed appointments unless 24 hours’ notice is given.
Length of Sessions:
50 minutes or 90 minutes.
Frequency of Sessions:
The frequency of sessions is optional at the Client’s discretion.
Fees:
Initial Consultation in person, on a one-to-one basis: 90 mins £60
Follow-on sessions
· on-line (via Zoom) or by phone: 50 mins £45
· in person, on a one-to-one basis: 50 mins £75
· on-line (via Zoom) or by phone: 90 mins £90
· in person, on a one-to-one basis: 90 mins £120

As a Resilience & Spiritual Life Coach and Counsellor, I will:
· Work in a private place.
· Ensure a viable Internet connection.
· Send a new invitation for each session with a secure password.
· Lock our virtual room.
· Conduct the session as closely on a one-to-one face-to-face basis as possible.
As a Client you will:
· Attend sessions in a quiet place with no distractions, which is secure and private and free of distractions.
· Ensure a viable Internet connection.
· Ensure an appropriate background.
· Ensure that you are suitably dressed.
Please note:
Clients have autonomy and must take ultimate responsibility for their own welfare and safety.
No recording of the session is permitted by Coach/Counsellor or Client.
No food or drink is to be taken into the session, apart from a glass of water.
Sessions last 50 minutes.
If Internet connection is lost and cannot be reinstated, sessions can be completed on the phone either by FaceTime where possible, or audio alone.
Privacy & Data Policy:
In line with GDPR requirements, it is important that I have your permission to store your data, as set out in my Private & Data Policy and Confidentiality Agreement.
While we cannot meet in person, please give me your consent using ONE of the following options:
Copy and paste the Statement below into an email and send back to me (via email).
Print, sign and scan the client consent section and email back to me.
Print, sign and post the privacy Policy to me at 1 Nigel Fisher Way, Chessington, KT9 2SN.
CLIENT CONSENT
I have read Howard Wright’s Privacy and Data Policy and I understand it. I agree that any personal details, including medical information, that I choose to provide may be recorded as described in this Policy by Howard Wright. I agree that Howard may contact me by phone and or/ email me in relation to our work together, or with relevant information concerning Howards practice. I understand that I may withdraw my consent at any time. I also understand and accept the terms in respect of fees, cancellations and missed appointments. I agree to working via 'Zoom' in the ways outlined in the additional document and accept that whilst every effort will be made to ensure that confidentiality of remote on-line sessions, this cannot be guaranteed.
